Below you will find our criteria and guidelines for requests for funding. You will also find our application in Word format. We look forward to hearing from you soon.

Large Donations - Annual Charity Recipient(s) - Application Process - $25,000 +

Phase I:

Letter of Intent - Charitable organizations are required to submit a Letter of Intent by December 1st that describes the proposed program or initiative. The Letter of Intent should include:
  • Organization name, address, phone number, contact name, charitable number, etc
  • Overview of organization
  • Title and brief summary of the program
  • Proposed timeline and estimated budget
  • Money raised to date and sources

Phase II:

Application - Charitable organizations are required to submit a formal application by February 15th.

Phase III:

Our Local Advisory Committee reviews proposals and makes a recommendation to the Board of Directors regarding the recipient(s) for the annual charity golf tournament and social.

Phase IV:

The Frog's Breath Foundation will issue a Press Release regarding the successful recipient(s).

Note: The Frog's Breath Foundation will entertain the possibility of medium size projects through the major recipient event by having multiple benefactors in a given year.
